Health and safety form

What is the purpose of each document?
Risk Assessment Form:

It is a form for any risks/hazards that you have seen that could become a threat to either the public, yourself or any other equipment that you might be using. It then contains contents of whether the action plain is Low, Medium or High meaning that actions could be done quicker or slower depending on how bad the risk could be. The Assessor and Line Manager both have to sign this document before anything can take place.

Adult Talent Agreement Release:

This form is an agreement between the photographer and the performer, both of them will have to sign this form due to the requirements that they both will follow. As long as they both agree to this form their will be no disagreements or regrets. This form also provides the model's age, name and number. Then it also has the Talent, Address and the date of the production.


Minor Talent Release:

This is much like the Adult Talent Agreement but it is for children under the age of 18. Also this contains the consent of a parent/guardians signature, saying that they accept the terms and conditions. It also has the phone number of either a parent or guardian. This would be if the photographer cannot get contact of the Model. 




Location Release:

This document is a consent of a property/location owner allowing you to use a place for a certain amount of time for how many days you would like it for. It is a place for you stay whilst you are doing work within that area. It asks for a signature from a crew member and that the owner has signed it as well. A phone number of both people will be left on the form for contact reasons and the address for the crew member for any damage done to the room will be his responsibility.
